Team Member Responsibitilites and Roles

Team Member

Responsibilities

(include but are not limited to)

Student*

  • Identifies personal needs, strengths, preferences and interests
  • Takes a leadership role in planning with supports
  • Participates in all planning activities
  • Identifies desired persons to be part of the transition team
  • Assumes IDEA rights at age of majority unless exempted by IEP team

Parent/Guardian*

Also:

  • siblings
  • friends
  • advocates
  • Provides information regarding student needs, strengths, preferences, and interests
  • Participates in referrals to transition programs and adult services
  • Assists in procuring Social Security identification cards, and transportation passes
  • Plans for long-term financial support, Social Security, trust funds, or other supports
  • Asks for assistance in obtaining community and residential services as needed
  • Provides opportunities for the student to try out adult roles and responsibilities 
  • Identifies the person who will coordinate the transition plan
  • Identifies friends and community members who can be part of the transition team

Special Education Teacher*

Collaborating with:

  • Related Services
  • Guidance counselor
  • Helps students identify postsecondary goals and obtain needed transition services
  • Identifies school or community agency personnel to be included in transition planning
  • Prepares the student and the family for leadership roles in IEP/transition planning
  • Writes the statement of needed transition services in the IEP transition plan
  • Coordinates transition services and activities in the IEP/transition plan
  • Provides information and assists families in developing referrals for adult services
  • Links the IEP to the student’s course of study and required testing
  • Collects and monitors information about student progress
  • Provides or obtains accommodations and supports for all education services

Regular Education Teacher*

  • Connects the IEP to the general education curriculum
  • Helps students identify postsecondary goals and needed transition services
  • Provides classroom instruction to support the student’s transition to adult environments
  • Collects and monitors information about student progress
  • Adapts curriculum and provides or obtains accommodations for regular education
  • Obtains or provides accommodations for state and regional proficiency tests

An individual who can interpret evaluations*

 

  • provides assessment information regarding student needs, interests, and preferences
  • provides assessment information regarding student strengths and aptitudes
  • interprets assessments and evaluations for the student and the family
  • identifies limitations of assessments and additional assessment  needs
  • works with the student and the family to identify assessment options

Representative of the Local Education Agency Familiar with the Curriculum*

  • Provides information about programs offered throughout the school
  • Identifies how the student can be included in general education programs
  • Assists in obtaining technology, accommodations, and supports
  • Helps identify how to address general education curriculum and competencies
  • Assists in obtaining accommodations and supports for student graduation
